Very simple multiple shot
A glass head was photographed upside down, the brain area was loaded with marbles, the marbles in the front was another shot, and the marbles inside the neck were glued in. Clearcut and both images assembled in photoshop. Done.
The idea has to be born, create a visual and proceed with an analysis on how to make it in reality.
